The story

Where the framework came from.

Most clinical frameworks are written in books before they are proven in the chair. The Structural Decision Framework was built the other way around.

For years at KYT Dental Services, Dr. Isaac Sun was making the same kind of decision over and over again, on different patients with different histories: is this tooth past the threshold where a filling stops being enough? Does this crack need treatment now, or watching? Is this missing molar redistributing force in a way that is going to cost the next tooth, or can it sit? Two patients would walk in with the same X-ray and walk out with very different recommendations. The reasoning behind the difference was real, but it had no name.

The Structural Decision Framework is that reasoning, named. Every tooth has a current value for three things: how much real natural structure is left, how much force it carries every day, and how long it has been carrying that load. Together those three variables predict where the tooth is heading. The skill is reading them.

Because the framework was developed inside a working practice, the 23 lessons published on this site are not theoretical. Each one describes a decision that has been made hundreds of times at KYT Dental Services in Fountain Valley. The framework is what runs the practice. This site is the patient-language version of the same thing.

A patient comes in with a back molar that has a large old filling and a hairline crack. They have been told, somewhere, that they probably need a crown. They want to know if that is really true.

At KYT Dental Services, this question is not answered by reaching for a default. It is answered by reading the tooth: how much real structure is left, what kind of force the patient puts on it (grinding? clean bite?), and how long the current arrangement has been holding. If the structure is past the threshold and the forces are high, a crown is the call. If the structure is still above the threshold and the patient is a light occluder, watching might be. The recommendation is the output of the reasoning, not the start of it.
